Despite the heartening success in restoring immune function in one AIDS patient by bone marrow transplantation from his twin, doctors would face formidible obstacles in trying that form of treatment on a broad scale against the disease.
Money and dedication could overcome some of these obstacles, but others depend on questions that are still unanswered by science and the art of medicine.
Transplants between unrelated people would be necessary if the technique were to make any major impact against acquired immune deficiency syndrome. But bone marrow transplants from donors unrelated to a patient are still highly experimental and the total number of such transplants has been minuscule compared with the number of AIDS cases.
A large increase in the number of marrow transplant patients would involve major expense, experts said, and it still remains to be proved that bone marrow transplantation can be effective when donor and recipient are unrelated. A Government study last year reported that fewer than 70 such transplants had been done altogether throughout the world and most of the patients lived only months afterward. None of these were done for AIDS. A Need for Caution Probably much that is valuable will be learned about AIDS through the present attempt, but Dr. Anthony Fauci, director of the National Institute of Allergy and Infectious Diseases, who reported the AIDS case at an international conference in Paris this week, emphasized the need for caution. He said there were no plans for widespread use of the treatment, which involves transplanting bone marrow, administering the antiviral drug suramin and transfusing white blood cells.
For the time being, he said, this therapy is contemplated only for AIDS patients who have identical twins. He also emphasized that the success in restoring the one patient's immune system has lasted only 10 months and that the treatment failed in two other patients.
Dr. Robert Peter Gale, of the University of California at Los Angeles, said his group has done four bone marrow transplants in gravely ill AIDS patients who have identical twins. The specialist said there was no unequivocal evidence of improvement. In collaboration with Dr. Fauci's group at the National Institutes of Health, Dr. Gale's group plans to do two more. In these cases, Dr. Gale said, a prior step will be the attempt to use drugs to eliminate all traces of AIDS virus from the patient before the transplant is done. Risks of Procedure
Experts estimate that about 1,000 bone marrow transplants are done each year in this country, most of them between relatives. In each case the patient must be protected rigorously against any infection for weeks afterward because he is extraordinarily vulnerable to microbes.
In addition, in cases other than transplants between identical twins, there is the risk that the cells of the transplanted marrow will attack the recipient's body as foreign. Such reactions are frequently fatal. There has been recent progress in reducing this risk by use of drugs and by removing the main cells that cause the reaction from the transplanted tissue. Some of these treatments, however, decrease the ability of the transplanted marrow to survive and prosper.
In contrast with the roughly 1,000 marrow transplants done every year, the Public Health Service estimates that there will be 26,000 AIDS patients alive in this country this year and that by 1991 this number will rise to 145,000.
The Government prediction is that there will be a cumulative total of 18,000 AIDS deaths in the United States by the end of this year. The cumulative death total from AIDS by 1991 is predicted at somewhere between 142,000 and 201,000.
The logistical and medical-scientific problems of using marrow transplants to make an inroad on this huge toll would be immense.
Dr. Robertson Parkman, a bone marrow transplant expert at Children's Hospital of Los Angeles, said the problems of facilities and supply of trained personnel could probably be overcome by a major effort like that mounted against polio before a vaccine was developed. Many Would Be Screened Because bone marrow transplantation is only feasible between persons whose tissue types match closely, large scale screening would be necessary to find donors for most AIDS patients. From studies on bone marrow transplantation for other purposes it has been estimated that only about 30 percent of patients are likely to have a close relative with suitable tissue.
Dr. C. Dean Buckner of the Fred Hutchinson Cancer Research Center in Seattle estimated that it is necessary to screen 10,000 persons to have a 50 percent chance of finding a suitable donor for any given patient.
Last year a Government panel headed by Dr. Brigid G. Leventhal of Johns Hopkins University considered the possibility of establishing a nationwide bone marrow registry to facilitate matching of potential donors with recipients. The panel concluded that local panels were more practical.
Dr. Leventhal explained yesterday that this was partly because the local panels can better stay informed on the status of all of its potential donors. She estimated that the tissue typing of a single patient or recipient costs $600 to $1,000. To multiply this by many thousands would involve an enormous investment for the tissue typing alone.

A boat docked in a tiny Mexican village. An American tourist complimented the Mexican fisherman on the quality of his fish and asked how long it took him to catch them.
"Not very long," answered the Mexican.
"But then, why didn't you stay out longer and catch more?" asked the American.
The Mexican explained that his small catch was sufficient to meet his needs and those of his family.
The American asked, "But what do you do with the rest of your time?"
"I sleep late, fish a little, play with my children, and take a siesta with my wife. In the evenings, I go into the village to see my friends, have a few drinks, play the guitar, and sing a few songs. I have a full life."
The American interrupted, "I have an MBA from Harvard and I can help you! You should start by fishing longer every day. You can then sell the extra fish you catch. With the extra revenue, you can buy a bigger boat."
"And after that?" asked the Mexican.
"With the extra money the larger boat will bring, you can buy a second one and a third one and so on until you have an entire fleet of trawlers. Instead of selling your fish to a middle man, you can then negotiate directly with the processing plants and maybe even open your own plant. You can then leave this little village and move to Mexico City, Los Angeles, or even New York City! From there you can direct your huge new enterprise."
"How long would that take?" asked the Mexican.
"Twenty, perhaps twenty-five years," replied the American.

"Afterwards? Well my Friend, That's when it gets really interesting," answered the American, laughing. "When your business gets really big, you can start selling stocks and make millions!"
"Millions? Really? And after that?" said the Mexican.

Experts: Sex slaves are the girls next door

NEW YORK (AP) — Raids conducted last week on 20 Northeast brothels uncovered more than 70 suspected sex slaves, exposing a long-ignored national problem found in towns large and small, with immigrants and U.S. citizens alike as victims, experts say.
"It's a very overwhelming subject for a lot of people to recognize that there is slavery at this time in our country," said Carole Angel, staff attorney with the Immigrant Women Program of the women's rights advocacy group Legal Momentum in Washington. "It's hard for us as humans to contemplate what this means."
Jolene Smith, executive director of Free The Slaves, a Washington-based organization dedicated to ending slavery worldwide, agreed that the idea of 21st century slavery was foreign to most people.
Americans are conditioned to believe that slavery was a thing of the past," Smith said. "We have to reeducate ourselves about this reality."
According to Angel, victims such as prostitutes are often handcuffed and hauled off along with the traffickers who coerced them into the sex trade.
That was not the case Tuesday when federal and local law enforcement raided brothels disguised as massage parlors, health spas and acupuncture clinics in six states and Washington D.C., arresting 31 people on trafficking charges.
They took more than 70 sex workers to undisclosed locations for questioning, and to provide basic services such as health care and food. Authorities said it might take weeks to get the Korean immigrants to trust them enough to discuss their ordeal.
"Human traffickers profit by turning dreams into nightmares," said Michael Garcia, U.S. attorney in Manhattan, where the majority of the traffickers face prosecution. "These women sought a better life in America and found instead forced prostitution and misery."
Yet Angel said the raids should not give the impression that trafficking is limited to immigrants, who are often coaxed to come to America for legitimate jobs only to be forced to work in brothels, sweatshops and restaurants to pay off debts of up to $30,000 to their traffickers.

The term pigtail appears in English in the American colonies in the 1600's to describe a twist of chewing tobacco. One of the steps in processing the tobacco was to twist a handful of leaves together to form a compact bunch that would then be cured (dried, either with or without smoking). The term "pigtail" was applied to the bunch based on its resemblance to a twisted pig's tail.
From the later 1600's through the 1800's, the term came to be applied to any braided (plaited, in British parlance) hairstyle. The British army also adopted a single pigtail or "queue" as its standard dress for long hair. Robert Louis Stevenson mentions "pigtail" referring to hair and then to "pigtail tobacco" in the first and fourth chapters of Treasure Island, respectively

The Oxford English Dictionary cites this usage as early as 1750s. Usually, it was just one pigtail. The further and later nautical use of pigtail refers to a short length of rope (1894.)
The term was also used as a derogatory reference to Chinese in the late 1800s. The pigtail was a mark of political enslavement to the Manchu dynasty, and westerners made fun of the Chinese hair style. The pigtail was abolished in China in 1911 when the Manchu dynasty was overthrown in favor of the Republic.

a nite amongst the palms
perhaps i am a child. a man i had never met before a short time ago read me like a book - i believe he is the only one who fully understands me, and that he did in just about 40 minutes. words were not exchanged at first. the dj played brazilian music and my body began to move. it was the first time i had really danced since angie's birthday nearly 5 months before, and that day i had someone i wanted to dance with, but instead danced with perhaps my best partner- the lady i just spoke of.
that nite in almunecar was different. i was alone, on my own & loving the freedom of being surrounded by people i did not know, a language i did not fully understand, and fully immersed in enjoying the rhythm of the music. my body moved - feet, legs, hips, shoulders, arms, and hair. i started yet another dance party. soon the people got off their chairs that were seated amongst palm trees. they laughed, smiled, and moved. it was a perfect nite.
a man dancing near the bar held a pipe. his body was atune to everything. i asked if i can take a foto. he stated yes but he wanted to take mine first. i am not sure what he shot b/c i was still dancing- hopefully he caught my glee. after the foto we spoke for a little. he said he hadn't seen anyone dance that well besides an african woman friend he met years ago. i told him i loved to dance - music was my life. it is the one thing i fully flow with. ...stating it here makes me certain that my life must flow along those beats- no medicine, no education, nothing without the beat.
he told me that i had the heart of a child but was ever-so mature. that i did not live in the here-and-now but somewhere in-between the past and the future. that my mind flows like the music, he can see it. i was grounded. it seems as if he came from somewhere so far away to tell me where i was (like ziggy in quantum leap). we shared names of books and music. we exchanged numbers and we were going to exchange a book, but it did not happen. i walked out and he stood from his seat blew me a kiss and said "have no fear, we will meet again." these words have been spoken before, by 2 other men that have strolled into my life. it is a strange occurrence. each time i think i have met them before. but each time they are completely different and teach me a new lesson. they teach me more in 40 minutes that others teach me in years...
